Construction and application of “Internet+ TCM nursing service” talent cultivation model in Zhejiang Province

Objective To construct an “Internet+ TCM nursing service” talent cultivation model in Zhejiang Province and to evaluate its application effect. Methods Based on the literature review and expert panels,the “Internet+ TCM nursing service” talent cultivation model was constructed in Zhejiang Province. A total of 63 trainees from 63 TCM and integrated traditional Chinese and Western medicine hospitals in the province were selected to carry out for 45 days of theoretical training and 9 days of clinical skills training. After the training,the training effect was evaluated by theoretical knowledge,TCM nursing skills,scenario simulation case assessment,trainees’ satisfaction,as well as a survey on the results of practice. Results The constructed talent cultivation model contains course contents including theoretical learning,skill training and emergency home treatment management and corresponding assessment programs. Sixty-three trainees successfully completed the course with the theoretical score of (92.24±3.64),the skill score of (97.13±2.53),and the situational case score of (95.78±3.02). Fifty-five trainees(87.30%) were assessed as “excellent” and 8(12.70%) as “good”. The trainees’ satisfaction about the overall module design of the project was(4.93±0.23) and 415 cases of “Internet+ TCM nursing service”were carried out. Conclusion The “Internet+ TCM nursing service” talent cultivation model in Zhejiang Province has strong applicability and remarkable training effect,which can provide references for the implementation of “Internet+ TCM nursing service” in other regions.

This research provides an overview of the perioperative nursing care for a patient with a massive renal cell carcinoma and Mayo Stage Ⅲ tumor thrombus who underwent a robot-assisted nephrectomy complicated by significant intraoperative bleeding and CO2 embolism. Key nursing considerations included multidisciplinary preoperative discussions,developing stepwise intraoperative emergency plans,establishing an integrated nursing workstation,and the optimization of patient monitoring. During the surgery,nursing efforts were focused on supporting the robot-assisted procedure,closely monitoring the patient’s condition,preventing intraoperative complications,initiating emergency plans as needed,actively managing major bleeding and CO2 embolism,and implementing structured positioning to minimize the risk of falls during the procedure. Postoperatively,the focus was on preventing secondary thrombus formation,managing active bleeding,and monitoring for liver and kidney ischemia-reperfusion injury. Through multidisciplinary interventions and meticulous nursing care,the patient was discharged after a 13-day postoperative recovery.

Research progress of symptom cluster in patients with acute coronary syndrome

Acute coronary syndrome(ACS) is a common type of acute and critical disease in Coronary Heart Disease(CHD). It is characterized by multiple and severe symptoms and rapid progression of the disease,which seriously threatens the life safety of patients. In this paper,the relevant studies on symptom cluster of patients with ACS were systematically searched at home and abroad,and the composition of symptom cluster,symptom assessment tools,symptom cluster establishment methods,influencing factors and intervention methods were reviewed,in order to provide references for nurses to manage ACS symptom cluster.

The best evidence summary for exercise rehabilitation in school-age children with malignant tumors

Objective To retrieve,evaluate and summarize the current evidence related to exercise rehabilitation in school-age children with malignant tumors,and provide references for clinical healthcare workers to guide children and their families to practice the plan for exercise rehabilitation. Methods The researchers conducted a systematical search in national and international databases,relevant guideline websites and professional society websites for related clinical decisions,recommended practices,guidelines,evidence summaries,expert consensuses,and systematic reviews for exercise rehabilitation in school-age children with malignancies since February 2017 to February 2022. The quality evaluation,extraction,and integration for evidence were conducted independently by 2 researchers,respectively. Results A total of 13 studies,including 3 guidelines,3 JBI evidence summaries,1 recommended practice,1 expert consensus and 5 systematic reviews,were included. There were 21 pieces of best evidence with 5 dimensions,including the purposes,participants,assessment,detailed programs,and management for exercise rehabilitation being summarized. Conclusion The study summarizes the best evidence of exercise rehabilitation in school-age children with malignant tumors. It is recommended for health professionals to fully consider the clinical situations to develop more detailed personalized exercise rehabilitation program based on the needs and preferences of school-age children with malignant tumors and their families.

Objective To explore the application of cluster nursing mode in perioperative period of elderly patients with hip fracture. Methods A retrospective cohort study design was used.Through convenience sampling method,50 elderly patients admitted to the hospital for hip fractures from January 2018 to December 2019 were selected as the control group. Meanwhile,47 elderly patients admitted to hospital for hip fractures from January 2020 to December 2021 were selected as the experimental group. The experimental group received cluster nursing,while the control group received routine nursing measures. The outcomes compared between the 2 groups included the effective rate of pain intervention,functional exercise compliance,and the incidence of complications. Results The effective rate of pain intervention(χ2=5.922,P=0.015) and functional exercise compliance(χ2=9.685,P=0.008)in the experimental group were higher than those in the control group. The number of perioperative complications(χ2=7.600,P=0.006) in the experimental group were lower than those in the control group. Conclusion The application of cluster nursing mode in the perioperative period of elderly patients with hip fractures can effectively control pain,improve functional exercise compliance,and reduce complications.

The assessment and influencing factors of hospital-acquired functional decline in elderly cardiac surgery patients

Hospital-acquired functional decline is common in elderly patients undergoing cardiac surgery,and it is closely related to a variety of adverse outcomes. This article reviews the current situation,adverse prognosis,assessment tools,assessment timing and influencing factors of hospital-acquired functional decline in elderly patients undergoing cardiac surgery,and also makes recommendations for further improvement of hospital-acquired functional decline,in order to provide a reference for clinical medical and nursing staff to explore suitable assessment tools,early identify the risk of hospital-acquired functional decline in elderly patients undergoing cardiac surgery and formulate personalized rehabilitation interventions.

Objective To describe the real psychological experience of patients waiting for a second kidney transplantation,so as to provide references for clinical medical staff to formulate targeted intervention strategies. Methods The descriptive qualitative research method was adopted. 15 patients waiting for a second kidney transplant were recruited from a hospital in Zhejiang Province by purposive sampling. Semi-structured interviews were performed to collect data,and qualitative content analysis was conducted to analyze the data. Results 4 themes were extracted,including the strong desire for a second kidney transplant,feeling guilty and worried for waiting a second kidney transplant,feeling deeply grateful for the medical staff’s support and being confidence for a successful second kidney transplant. Conclusion Clinical medical staff should attach importance to the psychological experiences of patients waiting for a second kidney transplant and construct patient-centered psychological intervention schemes by cultivating patients’ positive emotions,enhancing support level to improve their quality of life.

Objective To understand the status of Traditional Chinese Medicine(TCM) nursing service in general hospitals in Zhejiang province,and to provide reliable data for relevant departments to formulate supporting policies,so as to promote the sustainable development of TCM nursing service in Zhejiang province. Methods From August to November 2022,a whole-group sampling method was used to survey 103 general hospitals in Zhejiang Province using a self-designed questionnaire on the current status of TCM nursing services in general hospitals in the province. The questionnaire included 3 aspects:basic information of the hospital,current status of TCM nursing services and supportive needs for the development of TCM nursing services. Results A total of 103 valid questionnaires were collected,and the effective questionnaire recovery rate was 100%. Among the 103 hospitals,54(52.43%) are tertiary hospitals and 42(40.78%) are secondary hospitals,and 7(6.80%) are other hospitals. 91(88.35%) hospitals carried out TCM nursing services;73(80.22%) hospitals carried out no less than 5 TCM nursing technical items;34(37.36%) hospitals carried out no less than 4 TCM nursing technical categories;29(31.87%) hospitals served no less than 10,000 cases of patients per year with TCM care technology,and 24(26.37%) hospitals opened TCM nursing care clinics. The demand for support in the development of TCM nursing services is mainly related to the demand for policies,hospitals and training. Conclusion TCM nursing services in general hospitals in Zhejiang Province are well developed. However,there are still problems such as the insufficient development of TCM nursing clinics,unbalanced previous development of different levels of hospitals,shortage of TCM nursing talents,and imperfect quality supervision system,etc. In the future,TCM nursing support,talent training and standardized management should be further strengthened to promote the sustainable and steady development of TCM nursing in general hospitals.

Assessment and causes of low intake dehydration in elderly patients with cancer at admission

Objective To assess the status and analyze the causes of low intake dehydration in elderly patients with cancer at admission,and to provide references for early clinical recognition and prompt intervention. Methods From March 2021 to March 2022,elderly patients with cancer who had been hospitalized within 24 h in a tertiary A hospital in Tianjin were selected by convenience sampling method. The occurrence of low intake dehydration was judged by the calculated plasma osmolality,and the patients were divided into a normal group and a dehydrated group according to the plasma osmolality values,and the influencing factors for the occurrence of low intake dehydration in elderly patients with cancer were screened by logistic regression analysis. Results Among 706 elderly patients with cancer,the incidence of low intake dehydration during admission was 60.91%. Logistic regression analysis showed that comorbid cognitive impairment(OR=2.018),comorbid swallowing disorder(OR=1.611),Barthel score≤60 points(OR=2.572),serum albumin<35 g/L(OR=2.698) and duration of illness>6 months(OR=2.905) were influencing factors of low intake dehydration(P<0.05). Conclusion Elderly cancer patients who have a high incidence of low intake dehydration on admission and comorbid cognitive impairment,comorbid swallowing disorders,Barthel score≤60 points,serum albumin<35 g/L and duration of illness>6 months are more likely to develop low intake dehydration,and nursing staff should pay close attention to high-risk patients to identify and plan suitable interventions early to reduce the incidence of low intake dehydration.

Objective To construct the excellent talent cultivation model of TCM nursing and to evaluate its cultivation effect. Methods Based on the literature review and expert panels,the excellent talent training model of TCM nursing in Zhejiang Province was constructed. A total of 60 trainees in the province were selected to carry out a three-year training of excellent talents in TCM nursing in 2018. The final grades and a self-designed training effectiveness questionnaire were employed for evaluation,and a survey on the trainees’ satisfaction was conducted. Results The training model includes theoretical studies,clinical practice and skills training,as well as the development of teaching and research skills,and a corresponding assessment scheme. 54 trainees successfully completed the course with an overall completion score of (260.51±13.58). The trainees’ nursing ability and scientific research level of TCM nursing have been greatly improved,and their overall satisfaction score for the training was (4.83±0.42). Conclusion The TCM nursing excellent talent cultivation model in Zhejiang Province has achieved good results,which can provide references for the training of TCM nursing talents in other areas.

Construction of an intra-hospital emergency nursing program for patients with acute aortic dissection

Objective To construct an intra-hospital emergency nursing program for patients with acute aortic dissection suitable for the clinical scenarios in China,aiming to provide theoretical and practical guidance for carrying out evidence-based nursing practice. Methods Based on evidence-based methods,domestic and foreign literature was retrieved and summarized for the best evidence to form the first draft of intra-hospital emergency nursing program for acute aortic dissection. Delphi technique was used to conduct 2 rounds of correspondence among 30 selected emergency medical and nursing experts,and the final program was improved and established. Results 58 pieces of literature were included,and 95 pieces of best evidence were summarized. The first draft of the program was formed and 2 rounds of correspondence were conducted,with the recovery rate of the questionnaire of 100%,the expert authority coefficients of 0.869 and 0.868,the coefficient of variation of 0~0.17;the coordination coefficients of 0.473 and 0.613,respectively(P<0.05). The final intra-hospital emergency nursing programe for acute aortic dissection includes 3 level Ⅰ indicators,17 level Ⅱ indicators and 97 level Ⅲ indicators,including pre-examination triage,emergency rescue and nursing,and safe transferring. Conclusion The constructed intra-hospital emergency nursing programe for acute aortic dissection has detailed contents,wide coverage,strong scientificity and operability,which can provide references for emergency nursing practice.

Behavioral cue-based oral feeding pattern means that feeders make decisions and provide different oral feeding strategies through continuous and dynamic evaluation of the behavioral cues presented by premature infants. This paper reviewed the development background,development situation,theoretical basis,influencing factors and oral feeding readiness assessment,sucking quality evaluation,supporting strategy implementation method of behavioral cue-based oral feeding pattern for preterm infants in NICU. It aimed to provide the references to carry out the behavioral cue-based oral feeding pattern of premature infants at domestic NICU.

Objective To understand the feelings and experiences of medical staff on neonatal hospice care,and to provide a reference for the development of neonatal hospice care. Methods Descriptive research methods were used to conduct semi-structured interviews with 13 neonatal clinicians and clinical nurses,nursing managers,medical social workers,hospice care doctors and nurses,and the data was analyzed using targeted content analysis. Results 5 themes were extracted,including the needs of the dying newborn,the needs of the family of the dying newborn,the communication between doctors and patients,the timing of intervention for hospice care,and the multidisciplinary team of hospice care. Conclusion Medical staff should pay attention to the needs of dying newborns for pain reduction,comfort care,nutritional support,sense of security,and the needs of families to accompany children,emotional support,information support,professional guidance,understanding and respect,when conducting the neonatal hospice care of newborn babies. Effective and good communication between doctors and patients should be promoted. It is needed to consider the timing of intervention and the child’s disease condition and family will. Multidisciplinary collaboration is a good model for the development of neonatal hospice care.

Objective To translate the English version of Perioperative Thirst Discomfort Scale (PTDS) into Chinese,and to test the reliability and validity of the Chinese version of PTDS. Methods The English version of PTDS was translated into Chinese and back translated and modified for cultural adaptation according to the guidelines. A convenience sample of 316 perioperative patients at 4 tertiary hospitals in Fujian was investigated by the Chinese version of PTDS,in order to test its reliability and validity. Results The Cronbach’s α of the Chinese PTDS was 0.813; both the scale-level content validity index and the item-level content validity index were 1.Factor analysis showed 2 factors explaining 64.312% of the total variance. The score of PTDS was significantly associated with the thirst intensity score(r=0.681,P<0.001). Conclusion The Chinese version of PTDS has been proved to be reliable and valid.It can be used for measuring thirst discomfort in perioperative patients.

Effect of lavender aromatic therapy combined with emotional freedom techniques on insomnia cancer breast patients with heart and spleen deficiency

Objective To investigate the clinical nursing effect of lavender aromatherapy combined with emotional freedom techniques(EFT) on insomnia patients with breast cancer and heart-spleen deficiency.Methods A total of 140 patients with insomnia due to heart-spleen deficiency of breast cancer who met the inclusion criteria in the department of mammary gland of a tertiary hospital in Hangzhou from September 2020 to July 2021 were selected as the research subjects by convenience sampling method. Factorial design with 2 factors and 2 levels was adopted. According to the random number table method,the patients were divided into a control group(n=32),an aromatic group(n=33),an EFT group(n=32) and a combined group(n=32). The control group was given routine care of insomnia;on the basis of routine nursing for insomnia,the aromatic group was given lavender aromatherapy,and the EFT group was given EFT. The combined group was given the above 3 methods of comprehensive interventions at the same time,and the intervention period was 2 weeks. Pittsburgh sleep quality index(PSQI) was used to evaluate sleep quality within 24 hours and 2 weeks after intervention. Hospital anxiety and depression scale(HADS) was used to evaluate the improvement of negative emotions. The improvement of symptoms was evaluated by traditional Chinese Medicine(TCM) symptom score standard of deficiency of heart and spleen.Results Before and after the intervention,except the control group,the PSQI score,HASD score and TCM symptom score of the 3 groups were compared,and the differences were statistically significant(P<0.05). After intervention,the differences in PQSI score,HADS score and symptom score among the 4 groups were statistically significant(F=10.049,P<0.001;F=9.328,P<0.001;F=14.934,P<0.001;F=3.741,P<0.05),and the combined intervention of the 2 therapies was superior to any single therapy in the overall efficacy. The combined therapy has synergistic interaction effect on improving TCM symptoms of deficiency of both heart and spleen.Conclusion The combination of 2 therapies can effectively improve the sleep quality of breast cancer patients with insomnia of deficiency of both heart and spleen,and it has a magical effect in alleviating bad mood and relieving TCM symptoms.

Development and reliability and validity test of constipation risk scale for patients with severe craniocerebral injury

Objective The constipation score scale for patients with severe craniocerebral injury was developed,and its reliability and validity were tested,providing an intuitive,effective and simple tool for assessing the risk degree of constipation in patients with severe craniocerebral injury. Methods Guided by the framework of the theoretical study on the cause of constipation in patients with severe craniocerebral injury,a systematic literature analysis was conducted on this theme. A preliminary comprehensive pool of items was then formed,evaluated and supplemented through Delphi expert reviews. The preliminary investigation was conducted and the ambiguous items were revised. Finally,project analysis and exploratory factor analysis were employed to identify the best potential structure of the scale. The convenience sampling method was adopted to select 108 patients with severe craniocerebral injury admitted to the Department of Neurosurgery of a tertiary hospital in Jinzhong City,Shanxi Province from January to December 2019 as the research subjects,and a questionnaire survey was conducted to evaluate the reliability and validity of the scale. Results A pre-test questionnaire of 27 items was formed according to the results of expert letter consultation. 3 items that did not meet the standard were deleted after item analysis. 5 factors were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 66.347%. The 5 factors were named as intestinal peristalsis function,neuro-conscious dysfunction,excessive transport of intestinal water molecules,patient factor and medical personnel factor. The Cronbach’s α coefficient of the scoring scale was 0.908;the Cronbach’s α coefficient of each dimension was 0.830-0.957;the retest reliability was 0.879. Conclusion The constipation risk scale for patients with severe craniocerebral injury developed in this study has good reliability and validity. It can be used as an effective tool to evaluate the risk of constipation in patients with severe brain injury.

Evidence summary for the prevention and management of port-related venous thromboembolism in cancer patients

Objective To search,evaluate and integrate the relevant evidence on the prevention and management of port-related venous thromboembolism(VTE) in cancer patients,and to provide references for clinical practice. Methods We searched CNKI,the BMJ Best Practice,UpToDate,Cochrane Library,JBI Database,CINAHL,Embase,PubMed,China Guideline Clearinghouse(CGC),Scottish Interhospital Guidelines Network(SIGN),National Guideline Clearinghouse(NGC),American Intravenous Nurses Society(INS),National Comprehensive Cancer Network(NCCN),American Society of Clinical Oncology(ASCO),European Society for Medical Oncology(ESMO),and Registered Nurses’ Association of Ontario (RNAO) for relevant guidelines,systematic reviews,evidence summaries,recommended practices,and expert consensuses. The retrieval period is from the inception of databases to March 10,2021. Results A total of 12 articles were enrolled,including 4 clinical practice guidelines,5 systematic reviews,1 evidence summary and 2 expert consensuses. Totally 30 pieces of best evidence were summarized from 6 aspects,namely VTE risk assessment,catheter placement and maintenance,screening and diagnosis,prevention,management and treatment and health education. Conclusion This study summarized the best evidence on the prevention and management of port-related VTE in cancer patients. Nurses and nursing managers can select and apply the evidence according to clinical situation and patients’ wishes,so as to form a scientific,evidence-based,cancer port-related VTE prevention and management scheme,and improve nursing quality.

Qualitative analysis on the status and factors of the fear of physical activity in school-age obese children with asthma and their parents

Objective To explore the experience and feelings of school-age obese children with asthma and their parents on fear of physical activity based on fear-avoidance model,in order to provide a basis for the construction of targeted exercise intervention programs for children with asthma. Methods From October 2021 to January 2022,the purposive sampling was used to select 13 cases of school-age obese asthmatic children and their parents for semi-structured in-depth interviews in the pediatric asthma outpatient department of a tertiary A hospital in Yangzhou City. Data was analyzed by Colaizzi phenomenon analyzing methods. Results Combined with the fear-avoidance model,3 themes are summarized,including the unscientific cognition of physical activity(physical activity can induce a variety of uncomfortable symptoms;parents can’t tell if discomfort after physical activity is an acute attack of asthma;some parents are over-alert to obesity and force children to physical activity);the way to deal with the fear of physical activity(children actively avoid physical activity;parents limit children’s physical activity);the adverse consequences of fear of physical activity(the increasing sedentary behavior affects the exercise endurance of children,further aggravating their obesity and fear of physical activity;communication between children and peers was reduced,triggering a sense of inferiority,further aggravating the fear of physical activity). Conclusion The experience and feelings of fear of physical activity in school-age children with obesity and asthma and their parents were more complex with some unscientific cognitions. In clinical practice,measures should be taken to improve the cognitive level of children and their parents on appropriate and moderate physical activity,avoid excessive vigilance on exercise,reduce fear of physical activity,and promote scientific physical activity in children.

Investigation and analysis of factors related to colostrum feeding of premature infants in NICU

Objective To investigate the status of colostrum feeding of hospitalized premature infants in NICU and to explore the influencing factors. Methods From January 2020 to December 2021,851 premature infants who were born in the obstetrics department and admitted to NICU of a tertiary hospital in Wenzhou,Zhejiang province were selected as research subjects. According to whether colostrum feeding within 7 days after birth,they were divided into a colostrum feeding group and a non-colostrum feeding group. According to the proportion of colostrum feeding amount,the colostrum feeding group was divided into 3 groups,namely high,medium and low,and the colostrum feeding rate and influencing factors of premature infants were analyzed. Results A total of 374 premature infants(43.95%) received colostrum feeding. Among them,126 cases(33.69%) were fed with low proportion colostrum feeding,183 cases(48.93%) with medium,65 cases(17.38%) with high. Binary logistic regression analysis showed that the protective factors of colostrum feeding were education by neonatology nurses>2 times(OR=1.846),father’s support of breastfeeding(OR=1.349),education by obstetric nurses >2 times(OR=2.467) and mothers pump-ing milk≥12 times within 24 hours after delivery(OR=2.724). Maternal education level of junior middle school or below(OR=0.278) and maternal health problems(OR=0.462) were risk factors(P<0.05). Multiple logistic regression showed that,compared with the low and high proportion of colostrum feeding groups,the protective factors of premature infants receiving high proportion colostrum feeding were the mothers pumping milk≥12 times within 24 hours after delivery(OR=3.647),education by obstetric nurses >2 times(OR=1.906) and education by neonatology nurses >2 times(OR=1.535). Compared with the low and medium proportion colostrum feeding groups,the protective factors of preterm infants receiving medium proportion colostrum feeding were the mothers pumping milk ≥12 times within 24 hours after delivery(OR=2.995) and father’s support of breastfeeding(OR=1.203). Conclusion The colostrum feeding rate of hospitalized premature infants in NICU was low. Clinical medical staff should pay attention to the colostrum education of the families of premature infants separated from mother and infant,and assist mothers to establish regular milk pumping behavior after delivery.

Construction of neonatal hospice care service system

Objective To construct the hospice care service system for neonates for clinical practice guidance. Methods Based on humanistic nursing theory and comfortable nursing theory,a consultation questionnaire was developed via literature review and qualitative research during September to November 2020,and was distributed to 25 experts from 7 provinces and cities for Delphi expert consultation and neonatal hospice care service system construction. Results The questionnaire recovery rates of the 2 rounds of expert consultations were 100%;the expert authority coefficient was 0.852;Kendall’s concordance coefficients were 0.124~0.212 and 0.111~0.204 respectively (all P<0.001);the variation coefficients of the 2 rounds of item importance score were 0~0.34,0.07~0.24,and the variation coefficients of feasibility score were 0~0.39,0.08~0.24,respectively. The neonatal hospice care service system was formed with 4 first-level items,12 second-level items,and 38 third-level items,including multidisciplinary team cooperation,intervention timing,service recipients,service content. Conclusion The neonatal hospice care service system is scientific and feasible,which can provide certain guidance for the clinical practice of neonatal hospice care.

To summarize the nursing care of fecal microbiota transplantation in a patient complicated with pulmonary infection and pseudomembranous colitis after radical resection of rectal cancer. The main contents of nursing are as follows. The first stage is to prevent the aggravation of infection,including the observation and management of ileostomy and ostomy fluid,respiratory function exercise,early functional exercise,nutritional support,and disease observation. The second stage is the care of fecal microbiota transplantation,including preparation for transplantation,the key points of nursing during transplantation,and nursing after transplantation. After careful nursing,the patient has recovered and discharged from the hospital. After 6-month follow-up,the ileostomy fluid was controlled at a normal level of 560-1 050 ml/d,with the enhanced self-care ability and the improved symptoms of anxiety and depression,and the quality of life was improved.

Objective To study the influence of different temperature change curves on the duration of anesthesia and resuscitation in patients undergoing video-assisted thoracic surgery(VATS) for lung cancer,and to explore its implications for intraoperative temperature management and nursing,so as to provide evidence-based bases for clinical prevention of hypothermia and rapid rehabilitation. Methods The diagnosis and treatment data of VATS patients in 3 large hospitals in a province from January 2018 to September 2020 were retrospectively analyzed,and different intraoperative body temperature change curves of VATS patients were drawn. The effects of different curves on the duration of anesthesia resuscitation were compared. Results After screening,894 patients were included in the study. According to the different body temperature change curves during operation,they were divided into a “hypothermia without rising group”(281 cases),a “hypothermia rising group”(346 cases) and a “normal temperature group”(267 cases). The personal factors(age,gender,weight),anesthesia and operation factors (operation time,anesthesia time,infusion volume,bleeding volume,urine volume) and the rate(trend) of intraoperative body temperature change in VATS patients were not statistically significant(P>0.05). There were differences in the duration of anesthesia and resuscitation between the hypothermia without rising group and the other 2 groups(P<0.001). There was no significant difference in the duration of anesthesia and resuscitation between the other 2 groups(P>0.05). Conclusion The intraoperative body temperature of VATS patients decreased with the increase of operation time;timely and effective warming intervention given to VATS patients during the operation can prevent the occurrence of intraoperative hypothermia,maintain the normal body temperature after the operation,shorten the postoperative anesthesia recovery time and promote their rapid recovery.

Objective To assess the incidence and risk factors of postoperative nausea and vomiting(PONV) in liver cancer patients,and to provide references for clinical nursing intervention. Methods We used consecutive sampling method to select liver cancer patients who underwent liver resection at Zhongshan Hospital of Fudan University from October 2020 to April 2021. A total of 352 patients were prospectively studied. PONV was assessed after 2 hours,6 hours and 24 hours after surgery,respectively. Demographic data,disease information,and surgery information were collected to analyze the risk factors. Results The overall incidence of PONV in liver cancer was 48.30%(170 cases),of which 58.82%(100 cases) developed within postoperative 2 hours,20.00%(34 cases) developed between 3-6 hours after surgery,and 21.18%(36 cases) developed between 7-24 hours after surgery. Logistic regression identified the female,length of surgery,PONV history or motion sickness history,time of portal vein occlusion≥15 min and age<60 years old as risk factors of PONV. Conclusion The incidence of PONV within 24 hours in liver cancer was high.Nurses should pay much attention to those who were female at a younger age,with longer surgery,PONV or motion sickness history,longer time of portal vein occlusion,and take preventive measures to avoid or reduce PONV.

Methods We searched CNKI,CBM,UpToDate,PubMed,Cochrane Library,Joanna Briggs Institute,CINAHL,National Guideline Clearinghouse,National Comprehensive Cancer Network,American Society of Clinical Oncology,European Society for Medical Oncology for relevant clinical decisions,guidelines,systematic reviews,evidence summaries and expert consensuses. The retrieval period is from the establishment of databases to February 10,2022. Results A total of 17 articles were enrolled,including 4 clinical decisions,5 guidelines,4 expert consensuses and 4 systematic reviews. Totally 33 pieces of best evidence were summarized from 7 aspects,namely screening and evaluation,psychological intervention,instrumental and exercise intervention,non-hormone therapy,hormone therapy,related reproduction and training and follow-up. Conclusion This study summarized the best evidence for sexual health management in breast cancer patients. In clinical practice,nurses can personalize the application of evidence according to clinical situation and patients’ wishes to improve breast cancer patients’ sexual health.

Objective To retrieve,evaluate and summarize the best evidence of the resistance training for chronic obstructive pulmonary disease(COPD),so as to provide the evidence for clinical practice of resistance training for patients with COPD. Methods We searched BMJ Best Practice,UpToDate,Joanna Briggs Institute Library,National Institute for Health and Clinical Excellence(NICE),National Guideline Clearinghouse(NGC),Scottish Intercollegiate Guidelines Network (SIGN),Registered Nurses’ Association of Ontario (RNAO),Cochrane Library,PubMed,Medlive,CNKI,Wanfang Database,and CBM to collect relevant guidelines,expert consensuses,evidence summaries,recommended practice and systematic reviews on resistance training for patients with COPD. The retrieval period was from the inception of databases to March 2022. There were 2 researchers who evaluated the quality of the literature independently and extracted the data. Results A total of 15 articles were incorporated,including 8 guidelines,3 expert consensuses,1 recommended practice and 3 systematic reviews. A total of 18 pieces of best evidence including 8 dimensions were summarized,namely contraindication,training purposes,timing of training,training methods,training parameters,safety,health education,and effectiveness evaluation. Conclusion This study summarized the best evidence for resistance training in patients with COPD. It is recommended that health care professionals should formulate the individualized resistance training program based on the clinical situation and practical conditions,and fully consider the patient’s physical condition and willingness when applying the evidence.

Objective To form the Chinese expert consensus on cardiac rehabilitation nursing care,so as to standardize the work related to cardiac rehabilitation nursing. Methods The first draft was initially formed,based on a comprehensive review of domestic and foreign cardiac rehabilitation guidelines,expert consensuses,systematic reviews,original research and other literature,as well as related disease diagnosis and treatment plans,management consensuses and related literature. Results The draft was thereafter reviewed by a total of 37 experts in related fields. This consensus was finalized after 3 rounds of discussions and modifications. The expert consensus covers the following 4 topics,namely the work goals of cardiac rehabilitation nursing care,the qualifications,training,core competencies and responsibilities of cardiac rehabilitation nursing care,the components of cardiac rehabilitation nursing care,and quality control of cardiac rehabilitation nursing care,and the safety management of cardiac rehabilitation patients. Conclusion The consensus was formed on the basis of comprehensive expert opinions and relevant guidelines,consensuses and clinical research on cardiac rehabilitation,which can provide guidance for medical institutions to carry out cardiac rehabilitation nursing practice.

Congenital tufting enteropathy is rare to see in clinic,and it could be life-threatening due to intractable diarrhea in the early stage. The nutrition management was the most important and difficult part of nursing care. This study aimed to summarize the nutritional support experience of a case with congenital tufting enteropathy. In this case,we provide the personalized nutrition support based on nutrition screening and evaluation,and carry out parenteral nutrition support as soon as possible to help the child get fully potential of growth,as well as to get the metabolism balance. Besides,the timely enteral nutrition support was also used to improve the nutritional status of the child. Prevention and timely intervention for common complications were used to achieve clinical benefits. After a 60-day nursing care,the child’s condition was well-controlled with steady increasing in height and weight.

Risk analysis of readmission of preterm infants with bronchopulmonary dysplasia within 2 years of age based on the family participatory care model in the NICU

Objective To explore the influencing factors of re-admission of premature infants with bronchopulmonary dysplasia(BPD) within 2 years under the family participatory care model. Methods BPD premature infants with gestational age of 32 weeks or less admitted to the neonatal intensive care unit of a tertiary hospital in Wenzhou from January 2017 to December 2018 were selected as the research subjects. They were divided into a non-readmission group and a readmission group according to whether they were readmitted between the first discharge to 2 years old. Their clinical data during hospitalization and follow-up data under family participatory care were retrospectively analyzed,and the influencing factors of readmission were analyzed. Results A total of 178 BPD preterm infants were followed up,of which 98(55.06%) were not readmitted and 80(44.94%) were readmitted. Logistic regression analysis showed that the number of receiving nursing education was over 3 persons(OR=5.806) and severe BPD(OR=2.725) were the risk factors for BPD premature infants to be readmitted to hospital within 2 years of age(P<0.05). Receiving home oxygen therapy(OR=0.311) was a protective factor for read-mission(P=0.001). Conclusion During the period of carrying out family participatory care in the hospital,the number of receiving nursing education should not be too much,and the contents should be homogenized and standardized. Families of premature infants with moderate to severe BPD need to receive necessary training in home oxygen therapy,and the conducting of home oxygen therapy should be paid attention during discharge follow-up.

The application of cognitive interviews in the development of Patient Experience with Nursing Care Scale

Objective To explore the application of cognitive interview in the development of patient experience with nursing care scale. Methods According to the conceptual framework of patient experience with nursing care,the scale was developed by interviewing and expert consultations. By purposive sampling method,23 inpatients were selected as interviewees from June to July of 2020 in a tertiary hospital in Shanghai. 3 rounds of cognitive interviews were conducted to explore the respondents’ understanding of each item and their suggestions. The items of the scale were revised according to the results of the interviews and the expert group discussions. After the revision,we used convenient sampling to select 427 inpatients who were admitted to Zhongshan Hospital of Fudan University from May to June 2021 for testing psychometric properties,and the best potential structure of the scale was formed through program analysis and exploratory factor analysis. Results The first round of results showed that 8 items were revised due to "vague and unclear sentence"; the second round of results showed that an item was revised due to "incomprehensible keywords";the third round of results showed that the respondents were able to understand all the items. The final version of the scale includes 7 dimensions,namely,admission and discharge management,information and education,emotional support,observation and treatment,safety and privacy,response needs,communication and coordination,and the cumulative variance contribution rate is 70.785%. The Cronbach’s α coefficient of the scale is 0.945,and the average content validity index is 0.959 with good reliability and validity. Conclusion Cognitive interviews are helpful to the development of patient experience with nursing care scale,which effectively guides the revision of items,and can efficiently solve the problems of understanding differences and measurement errors in the process of scale development,so as to improve the scientificity,accuracy and applicability of the scale.

Reliability and validity of Chinese version of the Rheumatoid Arthritis Pain Scale

Objective To translate the Rheumatoid Arthritis Pain Scale(RAPS) into Chinese and to evaluate its reliability and validity. Methods The Chinese version of RAPS was translated,back translated and modified for cultural adaptation according to Brislin translation model. After the content validity was tested by an expert committee,a convenience sample of 236 rheumatoid arthritis patients in a tertiary hospital in Shanghai was investigated using the Chinese version of RAPS to test the reliability and validity of the scale. Results The Chinese version of RAPS includes 24 items,and Cronbach’s α was 0.969. Cronbach’s α coefficient of each dimension was 0.881~0.954.The scale-level content validity index was 0.97,and the item-level content validity index were over 0.80. Factor analysis showed that a total of 3 factors were extracted explaining 73.582% of the total variance,namely physiological,sensory and emotional cognition. Conclusion The Chinese version of RAPS has good reliability and validity,and it can be used to evaluate the pain level of patients with rheumatoid arthritis in Chinese cultural background.
